The social worker is a leader in supporting best practice models for child and youth services.   The duties  include providing a full range of social work services to adolescents and their families in the Ambulatory Outpatient Youth Mental Health division including students registered in the Fresh Start Day Treatment Program.  Services include, but limited to,  providing comprehensive bio-psycho-social assessments and family functioning; family therapy which may include  psycho-educational regarding mental health in adolescence, parental support, conflict resolution and facilitating referrals to community resources.  The position involves ongoing consultation and collaboration with an inter-disciplinary team to coordinate optimal services for youth and their families. Coordination of services with other team members including groups for families, parents or youth.  Intake and including coordination of the Fresh Start Day Treatment Program.  This involves the intake assessments of potential students, co-facilitating groups within the program and developing transition plans for students in conjunction with other team members. 

The position is Monday to Friday 830-1630, with some flexibility in hours for evening groups.

Qualifications

  • Master’s Degree in Social Work required
  • Member in good standing with the OCSWSSW
  • Minimum 3-5 years of experience working as a MSW in an Ambulatory/Outpatient Mental Health setting
  • Knowledge of the health care system and community resources for youth including alternative schools and day treatment programs.  Educate patients and families on available community services.

  • Strong team-building skills
  • Knowledge of the the principle tenants of DBT and CBT for complex mood and anxiety disorders
  • Extensive experience with family therapy
  •  Strong Crisis intervention and Risk Assessment skills
  • Strong knowledge of mood and anxiety disorders and current treatment interventions
  • Understanding of concurrent and addiction disorders
  • Knowledge of learning disabilities, ADHD and the relationship with mental health
  • CPI Crisis Intervention
  • Superior ability to manage multiple tasks and set priorities in a fast-paced environment
  • Commitment to student education
  • Demonstrated interprofessional collaboration knowledge and competencies
  • As part of our academic health sciences centre, you will participate in quality improvement initiative and continuing professional development activities that foster ongoing learning, professionalism, interprofessional collaboration and best practices in SW.  You will be required to preceptor SW students and act as a resource on an ongoing basis, in support of Sunnybrook’s full affiliation with the University of Toronto.
