This posting is for a temporary 1.0 FTE Registered Nurse (RN) position (ending August 2027). The incumbent is responsible for working as a member of the primary care team in the community and in the Sunnybrook Academic Family Health Team. The RN will primarily work with tenants in the local Toronto Senior Housing Corporation, including those with or at risk of dementia. In addition to the Sunnybrook Academic Family Health Team, the incumbent will work closely with the Community Psychiatric Services for the Elderly at Sunnybrook, as well as partners of the North Toronto Ontario Health Team as part of the Neighbourhood Care Team initiative. There will also be opportunities to work in the primary care outpatient clinic at Sunnybrook.   

 

Roles and Responsibilities
·    Assess patient health care needs (physical, emotional, psychological, and spiritual) to identify appropriate nursing interventions
·    Analyze findings of health assessments
·    Interpret patient health records
·    Monitor, record and report symptoms and changes in patients’ conditions
·    Collect data through therapeutic relationships with patients


Treatment/Management
·    Administer medications and treatments as prescribed by a physician/nurse practitioner
·    Administer vaccinations
·    Blood pressure assessments
·    TB skin tests
·    Foot assessments / Foot care


Education/Advocacy
·    Help patients to identify and use health resources, including those available in the community and from community agencies
·    Involve patients in decisions about their own health
·    Encourage patients to take action for their own health
·    Initiate health education and other activities that assist, promote and support patients as they strive to achieve the highest possible level of health

 

Referrals/Collaboration
·    Collaborate with members of an interdisciplinary team to plan, implement, coordinate and evaluate patient care in consultation with patients and their families
·    Consult and interact with other service providers as needed

 

Administration/Reporting
·    Coordinate and expedite patient flow by triage
·    Provide Health Education in person and by telephone.
·    Arrange follow up appointments as necessary
·    Collect accurate and comprehensive patient-related information and documenting same in the patients' Electronic Medical Record.
·    Participate in Quality Improvement Initiatives
·    Operate and monitor medical apparatus or equipment and ensure strict adherence to sterilization procedures
·    Handle and dispose of specimens and/or objects, and practice general safety procedures at all times
·    Monitor supply levels and/or expiration of medical and clinic supplies, and order supplies as needed from approved vendors

 

Qualifications/Skills
·    Current registration with the Ontario College of Nurses
·    Registration in good standing with the Ontario College of nurses
·    Primary Health Care / Community Care experience preferred
·    Experience with working with persons with dementia
·    Excellent oral / written skills
·    Strong clinical skills
·    Ability to adapt to constantly changing clinical situations
·    Autonomous and able to function with minimal supervision
·    Ability to work effectively with all members of the inter-professional team
·    Ability to prioritize, manage time effectively and be flexible in a very active work environment
·    Basic CPR, BLS certification up to date within the last 2 years
·    Experience with EMRs, Telus Practice Solutions in particular is an asset
·    Have a valid Ontario Driver’s license and access to a vehicle
