Summary of Duties

The Registered Nurse provides holistic, high quality and safe care to patients prescribed by the physician and in compliance with the College of Nurses of Ontario. The nurse will assess, plan, implement and evaluate patient care in compliance within the scope of practice for multiple services. The nurse will receive patients into clinics, obtain and document nursing assessment, vital signs, health and social history as applicable. The nurse is responsible for providing direct care for patients as prescribed by the physician, monitor patients and apply interventions as required in accordance with standards of the College of Nurses, the organization and Person Centred Care philosophy. The Nurse triages patient phone calls, provides telephone advice, and arranges for follow-up appointments as necessary and documents accurately in the appropriate documentation tool. Collaborates with patients, families and members of our interdisciplinary team to plan, implement, and evaluate patient primary care plans. Consult and liaise with external service providers as needed to support the development of coordinated primary care plans. The nurse provides education/counseling, and documents information according to appropriate standards. The nurse works closely with PAAs and physicians to support patient triaging, clinic flow, and scheduling.

 

Qualifications / Skills:

  • Current Registration with College of Nurses of Ontario
  • Baccalaureate degree in nursing preferred
  • ACLS preferred
  • Current Basic Cardiac Life Support required
  • Recent Acute and Ambulatory Care nursing experience required.
  • Experience with a wide variety of differed services including Family Practice is an asset
  • Demonstrated commitment to Patient-Focused Care concept and principles
  • Ability to set priorities in a team-oriented, fast-paced environment
  • Ability to handle stressful environments, in a positive manner
  • Excellent interpersonal, verbal and written communication skills
  • Demonstrated ability to work well both independently and as part of a multi-disciplinary team
  • Demonstrated satisfactory attendance and punctuality required
  • Computer literacy and typing required
  • Able to use  Accuro,Telus Practice Solutions, Sunnycare, Excel, PowerPoint,  Microsoft office, & BMS
  • IV insertion required
  • Phlebotomy
