The Garry Hurvitz Brain Sciences Centre (“GHBSC”) will be a first of its kind in Canada, unifying the treatments for brain illness in a state-of-the-art collaborative space. The Yuval & Lori Barzakay Brain Health Clinic, located within the GHBSC at Sunnybrook Health Sciences Centre, is seeking two (2) experienced Registered Nurses (RN) specializing in the ambulatory care of patients living with neurological disorders and diseases.
Summary of Duties:
The Yuval & Lori Barzakay Brain Health Clinic’s RN role offers the opportunity to contribute to a pioneering facility dedicated to advancing brain health through integrated and innovative approaches. The duties of the RN team include but are not limited to:
- Comprehensive health assessment, development of supportive & therapeutic patient & family relationships, management of neurological symptoms & treatment side effects, teaching & coaching, and support with clinical decision making & patient advocacy.
- Provide patient education on medication use, side effects, and self-management strategies.
- Provide guidance on when to seek emergency care.
- Collaborate with the interprofessional team and other health care providers to prioritize urgent issues.
- Engagement with Primary Care Providers (PCP) to support and reinforce shared care model of care.
- Coordinate with pharmacies and insurance companies for prescription and drug approvals.
- Reinforce treatment plans, follow-up care and link to community supports.
- Educate caregivers on how to assist patients with symptom management at home.
- Assisting HCPs in performing a variety of outpatient procedures (i.e. lumbar puncture).
- The nurse will be responsible for the on-going assessment and care of the patient during the procedure and will be expected to provide discharge teaching with patients/families in preparation for discharge home.
- Perform IV insertions for neurological treatments, monitor patients for adverse reactions during and after IV infusions, educate patients on infusion expectations, potential side effects, and post-infusion care.
- Performing all aspects of patient care processes prescribed by Physicians &/or Nurse Practitioners in compliance with the College of Nurses of Ontario (CNO).
Qualifications/Skills:
- Current registration with the College of Nurses of Ontario (CNO) in good standing.
- Bachelor of Science in Nursing (BScN).
- Current B.C.L.S. required.
- Recent experience in the hospital-based care of patients with neurological disorders (inpatient or outpatient), minimum two (2) years, required.
- Emergency department (ED) nursing experience preferred, minimum two (2) years, preferred.
- Demonstrated ability to take professional accountability for leading, organizing, providing and delegating care activities for the units, patients and families.
- Demonstrated advanced triage, critical thinking, communication, problem solving, conflict resolution and interpersonal/team building skills.
- Fosters ongoing learning for staff and students with a commitment to preceptorship
- Integration of research findings into practice, participate in unit projects or organizational initiatives such as i-lead and best practice champion.
- Demonstrated commitment to adaptability, continuous learning and evidence-based practice.
