Position Summary:
As a member of the Community Integration, Partnerships and Outpatient Care portfolio, the Program Manager is a leader who will support the development, delivery and ongoing management of transitional and community care programs aligned to the portfolio. Reporting to the Director, Community Integration and Outpatient Transformation, and in collaboration with internal and external partners, the Program Manager will oversee Sunnybrook programs supporting transitions of care, such as the Sunnybrook-to-Home Program and the Centralized Community Health Professions Team, as well as providing management support to the Pine Villa Transitional Care Site. 

Key Responsibilities:
•    Provide operational leadership and management level oversight of multiple transitional and community care programs in the Community Integration, Partnerships and Outpatient Care portfolio
•    Serve as operational point of contact for transitional programs in the portfolio
•    Manage relationships and contracts with partners to ensure relevant standards and accountabilities are consistently met
•    Collaborate with portfolio and program leadership, Decision Support and Finance in the development of performance measures related to transition of care
•    Analyze data and provide regular updates to internal and external leaders, including Ontario Health
•    Work with internal programs and external partners to identify, implement, maintain and evaluate opportunities to optimize patient transitions 
•    Collaborate with professional practice team to ensure standardization and optimization of professional roles in support of transitional programs in the portfolio  
•    Supervise, support and manage health professional staff at Pine Villa and the Centralized Community Health Professions Team
•    Management of the operations, supplies / equipment, and maintenance of the building at Pine Villa to ensure smooth functioning of the site
•    Chair program and care related site committees and working groups
•    Identify, mitigate and appropriately escalate project risks to ensure program success to Director/VPs
•    Report/monitor deliverables across multiple programs 
•    Promote a positive and progressive image for Sunnybrook with both internal and external stakeholders

 

Education and Experience 
•    Masters degree required (preferably in a health related discipline, health administration and/or business)
•    At least five years of recent or related progressive health care and management experience 
•    Experience in a leadership role managing teams involved in clinical care or complex projects and demonstrated project management experience is an asset
•    Proven management skills including financial, human resources, quality improvement, project planning and risk management skills
•    Have a strong understanding of the health care environment including funding methodologies and hospital data structures (clinical and financial)
•    Excellent interpersonal skills, including communication, conflict resolution, organization skills, critical thinking, decision making and problem solving
•    Demonstrates ability to work in a team environment and collaborates with others in assisting with the delivery of care to patients
