Summary of Duties

To provide a full range of social work services to adult Mental Health patients and their families. Services include psycho-education regarding mental illness, mediation and assistance in resolving conflict; ongoing consultation and collaboration with colleagues to coordinate optimal services for patients with an interprofessional approach; resource counselling – referrals to appropriate community supports; optimal discharge planning and follow up care arrangements.

Qualifications/Skills

    • Masters Degree in Social Work required
    • Member in good standing of the Ontario College of SW
    • Minimum 1 year of experience working as a MSW in Mental Health
    • Commitment to Patient Centered Care
    • Excellent communication and organization skills
    • Strong team building skills
    • Excellent assessment, interviewing and problem solving skills
    • Knowledge of the health care system and community resources
    • Superior ability to manage multiple tasks and set priorities in a fast-paced environment
    • Commitment to student education
    • CPI (Non-Violent Crisis Intervention) an asset
