Position Summary

Sunnybrook Health Sciences Centre is a proud member of the North Toronto Ontario Health Team. Working in close collaboration with our OHT members, we are recruiting for Community Ambassadors, who will play a crucial role in building trusting relationships and actively engaging with community members in North Toronto. The goal is to work with community members to provide support connecting them to primary care, and to raise awareness regarding the importance of having a primary care provider. The Community Ambassadors will act as a point of contact between the North Toronto Ontario Health Team and its community members, and will help guide community members regarding their health care navigation needs.

 

Key Responsibilities

  • Working collaboratively with members of the North Toronto Ontario Health Team, the Community Engagement Committee and the Community Advisory Council, provide input into the development of an effective outreach plan to support primary care attachment in North Toronto.  
  • Support the execution of the outreach plan and conduct grass root community outreach activities. Outreach activities will be done primarily in the community. Outreach may also be required on-site at Sunnybrook Campuses (mainly Bayview).
  • Work under the guidance and support of Community Engagement Project Manager to engage members of the community to connect them with primary care, and support navigation to other health care information  
  • Participate in the collection of trends, outreach activity statistics and project evaluation activities
  • Participate and support in educational sessions, town halls and other health promotion activities, including set up and take down of equipment and materials for related events
  • Perform other duties as assigned within the scope of the job

 

Qualifications

  • Strong knowledge of Davisville and the North Toronto area.
  • Well-connected with local schools, community agencies, and residents with demonstrated knowledge, skills and abilities in social networks in community
  • Demonstrated experience and knowledge of asset based community development approach to outreach activities
  • Demonstrated knowledge of community resources, health promotion and the impact of social determinants of health and public health policies
  • Experience in community-facing roles (e.g., outreach, health promotion, customer service).
  • Willingness to learn and communicate health-related information, including chronic conditions and primary care services.
  • Strong interpersonal skills, with tact and cultural sensitivity, with ability to handle confidential information with discretion.
  • Self-motivated, organized, and able to work independently as well as part of a bigger team
  • Ability to work evenings and some weekends
  • Experience in group work and program development or event planning, this includes formal work or social experience
  • Ability to implement, monitor, and evaluate outreach program an asset
  • Excellent communication skills (written and verbal) and group facilitation skills
  • Multilingual abilities (e.g., Russian, Mandarin, Spanish, Tagalog, Korean, Farsi) are an asset but not required.
  • Ability to use of computer and various software applications including word processing, spreadsheets, presentations, email and social media an asset but training can be provided
