Summary of Duties:

The Patient Care Manager is responsible for the overall management of the patient care unit and the quality of care provided.  The manager leads and collaborates with the interprofessional team to achieve resident and family centred care. The role includes creating and maintaining a supportive environment for patients, families and staff, providing team leadership, facilitating staff and team development, managing budgets and resources, and initiating and monitoring quality improvement initiatives.  The PCM works with the Veterans Program leadership team to achieve program goals.

 

The PCM is also accountable for ensuring that care requirements are met, and that patient care issues, problems and concerns identified by patients and their families are addressed. The LGSE and LGSW units specialize in the care and quality of life for patients with moderate dementia.

 

Qualifications/Skills:

  • Bachelor Degree required, Master’s degree preferred
  • Registration in good standing with an Ontario Regulated Health Professional College required, Nursing Preferred
  • Minimum 2 years geriatric experience including dementia care required
  • 2 Years Management / Leadership experience
  • Experience leading within a unionized environment
  • Superior leadership, communication, team building and decision making skills
  • Superior oral & written communication skills
  • Demonstrated coaching and empowering style of management
  • Demonstrated interpersonal skills
  • Proven ability to work in a team environment and collaborate with others
