Summary of Duties

A Registered Nurse (RN) who is part of the Heart Function (HF) clinic provides holistic, high quality and safe care to patients prescribed by the physician and in compliance with the College of Nurses of Ontario. They provide direct care to outpatients with complex care needs within the HF clinic. Although the position is based within the Schulich Heart Program, it will also require collaboration with the Transfusion Medicine team to provide occasional cross-coverage while utilizing the transfusion medicine clinic space for services related to HF clinic patients.

The main functions of the RN include, but are not limited to:
•    Receives patients into clinic, obtains and documents nursing assessment, vital signs and assists with the administration of quality of life questionnaires (i.e. KCCQ-12).
•    Receives and triages referrals into the HF clinic and assists in updating the administrative database.
•    The nurse is responsible for providing direct care including and not limited to performing ECGs, intravenous therapy, iron transfusions, venipuncture and vascular access/ device selection in accordance with standards of the College of Nurses, the organization and Person Centred Care philosophy.
•    The nurse will assist in provision of education/counseling, and documents information according to appropriate standards. 
•    As a member of the team, the successful candidate should be willing to provide the occasional cross coverage to the transfusion medicine ambulatory clinic when required. 
•    The nurse will assist patients in both the ambulatory and inpatient settings in onboarding of the Medly Application.
•    The nurse will be responsible for monitoring and updating the Medly dashboard and database, contacting patients in response to alerts, maintenance of equipment and liaising with the Medly application support team as required. 
•    The nurse will assist in filling out referral forms, requisitions for investigations, and Employee Assistance Program applications. 
•    The nurse will collaborate with the interprofessional team in the management of heart failure patients and their families throughout the trajectory of their care to help address and manage physical, emotional, practical, psychological, informational, social, and spiritual needs. 
•    The nurse will assist in supporting patients and their families beyond the clinic environment through phone assessments if needed as it is at times critical for the supportive management of the patients with heart failure.
•    The nurse will assist the interprofessional team in patient education and care coordination. 

Qualifications/ Skills

•    Required current certification of registration from the College of Nurses of Ontario 
•    Current BCLS certificate is required
•    Coronary Care Course I & II is an asset
•    A minimum of two (2) years related clinical experience in management of heart failure is strongly preferred 
•    Demonstrated clinical expertise in cardiology is an asset
•    Demonstrated clinical expertise in geriatric and chronic conditions preferred 
•    Demonstrated high level of experience, knowledge and skill in performing ECGs, intravenous therapy, iron transfusions, venipuncture and vascular access/ device selection.
•    Experience in ambulatory care nursing is an asset
•    Competent computer skills required
•    Must be flexible, adaptable, and have the ability to prioritize and manage time effectively 
•    Excellent interpersonal, oral and written communications skills necessary
•    Demonstrated commitment to on-going education and professional development important
•    Commitment to the quality care of our patients, evidence based practice and a clear understanding of professional standards
•    Ability to integrate and apply critical thinking skills to patient care within scope of practice
•    Ability to function independently and as a part of the interprofessional team
•    Demonstrated experience in conflict management
•    Be open to meeting the ever changing needs of patient care and organizational objectives
