  • Reporting to the Manager of Patient Flow, the Patient Flow Specialist will be responsible for the following:

    • Coordinate patient flow activities across the hospital, including admissions, transfers, and discharges both within the organization and externally with other acute care organizations.

    • Work closely with clinical teams, nursing units, physicians, and external partners to support timely patient movement and continuity of care.

    • Identify and assist in resolving barriers that delay patient discharge or transfer, including communication gaps, process issues, and resource constraints.

    • Monitor patient flow trends and compile daily summaries, operational updates, and ad hoc status reports for Senior Leadership to support informed decision-making.

    • Educate staff on admission, transfer, and discharge processes

    • Managing surge and multi-casualty incidents

    • Collaborate with patient care units (including the OR) to optimize patient flow

    • Participate in DNLP presentations and leadership development initiatives to support professional growth and the development of future healthcare leaders

     

    Qualifications/Skills 

    • Registered Nurse currently registered with the Ontario College of Nurses

    • Four year degree in a relevant health profession or equivalent, completed or in progress

    • 3-5 years direct patient care experience in acute care

    • Previous patient flow experience preferred

    • Demonstrated ability to work effectively with multidisciplinary teams (e.g., MDs, other health professionals)

    • Excellent communication, facilitation, leadership, decision-making and organization skills

    • Demonstrated experience using Bed Management Systems, patient tracking systems, and Microsoft Office applications including Excel, Word, and Outlook.
    • Demonstrated commitment to Patient Focused-Care concept and principles
    • Ability to work effectively in a fast paced environment with minimum direction

    • Demonstrated ability to work in a collaborative and respectful fashion
