Sunnybrook Health Sciences Centre is seeking an experienced and innovative clinical pharmacy leader to serve as Clinical Coordinator, Intensive Care & Surgery Pharmacy Services. Reporting to the Professional Practice Lead/Manager, Pharmacy, the Clinical Coordinator provides leadership for pharmacy practice across critical care and surgical programs while maintaining an advanced clinical practice focused on optimizing medication-related patient outcomes. Summary of Duties  The Clinical Coordinator is responsible for advancing clinical pharmacy practice within Sunnybrook's Intensive Care and Surgical Programs through a combination of:

  • Advanced direct patient care
  • Professional practice leadership
  • Staff mentorship and education
  • Quality improvement and research
  • Program development and administration

As the clinical practice leader for these specialty areas, the Coordinator works collaboratively with pharmacists, physicians, nurses and the interprofessional team to ensure safe, effective and evidence-informed medication use throughout the continuum of care.

 

Key Responsibilities include leading in Clinical Practice Leadership & Direct Patient Care; Professional Practice Leadership, Education & Mentorship; Teaching & Academic Contributions; Research, Quality Improvement & Innovation; and Administration & Program Development

 

Qualifications/Skills

 

REQUIRED: Licensed Pharmacist in good standing with the Ontario College of Pharmacists (Part A Register). Must hold one of the following qualifications: either a master’s degree in a pharmacy or related discipline, or relevant Accredited Canadian Pharmacy Residency Year 2 Residency (ACPR-2) or a Pharm D as a post-baccalaureate or second professional pharmacy degree (not an entry to practice Pharm D). Applicants must have a minimum of 5 years of hospital pharmacy practice experience (10 years or more preferred) with demonstrated expertise in critical care or surgical pharmacy practice. Applicants must have excellent communication, collaboration and leadership skills.

PREFERRED: 5 or more years of practice in Intensive Care Pharmacy Practice. Board of Pharmacy Specialties (BPS) certification or equivalent advanced credentialing. Record of scholarly achievement, quality improvement and/or clinical research and experience teaching pharmacy learners and healthcare professionals.
