Sunnybrook opened in 1948 as Sunnybrook Military Hospital. With a long history of caring for Canada’s Veterans, Sunnybrook is home to one of the largest Veteran care facilities in Canada. Canadian Armed Forces personnel requiring care as a result of their service to our country also receive specialized care at Sunnybrook. We are currently creating a regional care and service hub to ensure that everyone who has served in the Canadian Armed Forces has timely access to health care that meets their needs.
We are seeking a dedicated and experienced health care navigator and project lead to develop and implement a regional navigation hub for Canadian Armed Forces and Veterans in the GTA region. This will involve creating a full inventory of care and services, including access processes; partnering with other health care and service providers/funders; and identifying gaps in care and services, with solutions co-designed with military personnel and Veterans to fill these gaps.
The candidate must have an enthusiastic approach to continuous learning, together with excellent leadership, organizational, and communication skills. The ideal candidate will have experience in health care delivery, management, and healthcare model development.
Requirements
- Clinical background in nursing, occupational therapy or social work.
- A minimum of 5 years of clinical experience within areas such as emergency care, geriatrics, medical/surgical specialties, addictions management, brain injury/trauma rehabilitation, mental health/psychiatry or chronic pain management is preferred.
- A minimum of 2 years experience in health care navigation.
- Experience in health care model development is an asset.
- Critical thinking skills with the ability to navigate complex situations as needed.
- Strong organizational and project management skills.
- Exceptional interpersonal, verbal, and written communication skills.
- Strong proficiency (intermediate to advanced) utilizing Microsoft Office applications.
