Registered Nurse - Home Dialysis - Temporary Full-Time 2026-18815

Summary of Duties

The RN is responsible for assessment, planning, implementing, advocating and evaluating patients with end stage kidney disease initiating and being maintained on home dialysis modalities.

As a member of the inter-professional team providing person-centred care, responsibilities include but are not limited to:

• Plan and provide personalized care with each patient;
• Perform and document multiple assessment and follow-up activities;
• Provide health teaching & family counselling and training specific to the patient’s modality choice and ongoing support as a primary nurse or part of the team;
• Provide modality education support for home modalities including home visits;
• Analyze clinical data and utilize critical thinking to make decisions regarding required care/interventions related to patient’s clinical status;
• Participate in weekly home dialysis clinics and provide follow-up care as required
• Liaise with vendor and community partners;
• Act as a mentor to colleagues and others to improve/support nursing knowledge related to home dialysis modalities, including Transitional Care Unit;
• Participate in the advancement of patient care through inter-professional rounds, committees, quality initiatives and practice-based research;
• Utilize Nephrology EMR and other computer applications to facilitate communication, documentation, recording and tracking of patient assessment, education and plan of care.

Qualifications/Skills

• Current Registration with the College of Nurses of Ontario in good standing
• Current B.C.L.S. Certificate required
• Baccalaureate degree in Nursing, preferred
• Certification in Nephrology CNeph(C), preferred
• Experience with Peritoneal Dialysis and venipuncture preferred
• Recent (3 year) Hemodialysis experience required
• Recent adult education program development and teaching experience
• Ability to integrate best practice guidelines into hospital policies and procedures
• Knowledge of MS Office suite applications
• Excellent patient assessment; communication; organizational; team building and conflict resolution skills
• Demonstrates understanding and complies with patient and staff safety policies
• Commitment to person-centred care concepts and a demonstrated ability to work as a member of an inter-professional team
• Must have a valid driver’s license and have access to personal vehicle in order to deliver care in a home setting
