We currently have a Regular Part-Time (0.4 FTE) opportunity for a Social Worker in the ED1 Team Department at our Bayview Campus.

Hours of Work: Weekday: Evenings , 8hrs; as per schedule

Weekend: Evenings, 8hrs; as per schedule

Summary of Duties

To provide a full range of psychosocial assessment, intervention and care planning to patients admitted to Surgical Oncology.

Responsibilities include:

  • Psychosocial assessments, counselling, transition planning, advocacy and participating as a key member of the care team
  • Participate in the advancement of patient care through daily inter-professional rounds, unit-based and organizational committees, quality initiatives and practice-based research
  • Active member of the social work team and participate in team meetings and SW led initiatives

Qualifications/Skills

  • Masters of Social Work Required
  • Member in good standing of OCSWSSW
  • Minimum of two years’ experience in a health care setting required
  • Oncology experience preferred
  • Excellent assessment and intervention skills
  • Knowledge of ACP and GoC skills and framework in working with this population
  • Collaborative practice competencies
  • Knowledge of the health care system, its legislation and community resources
  • Demonstrated problem solving and conflict management skills
  • Superior facilitation skills
  • Commitment to Student Education and IPE activities
  • Commitment to Person Centred Care
