Summary of Duties

To provide a full range of social work services to General Internal Medicine patients and their families on the medical units.   Services include psychosocial assessments, counselling, discharge planning, linkage to community resources, facilitating Advance Care Planning Conversations and addressing complex care issues.

 

The successful candidate will participate as a key member of the patient care team on General Internal Medicine.

 

Qualifications/Skills

  • Master’s Degree in Social Work required
  • Member in good standing of the Ontario College of SW and SSW
  • Minimum two years of experience working as an MSW in a Hospital setting; experience working with GIM, oncology and surgical patients preferred
  • Commitment to Person Centred Care Best Practices
  • Excellent communication, organization skills and time management skills
  • Strong teambuilding skills and conflict resolution skills
  • Excellent assessment, interviewing and problem solving skills
  • Knowledge of the Health Care Legislation, health care system and community resources
  • Strong ability to coordinate community resources related to pain and wound management and palliative care support.
  • Superior ability to manage multiple tasks and set priorities in a fast-paced environment
  • Knowledge of and past clinical experience in Inter Professional Collaboration and InterProfessional Education
  • Commitment to student education
  • Interest in exploring Academic Practice opportunities that address Patient and Family Care across the continuum of Health Care
