Occupational Therapist - ED One Team - Regular Full-time 2024-10362

We currently have a regular full-time (1.0 FTE) opportunity for an Occupational Therapist as part of the ED One Team in collaboration with the Emergency Department (ED) at Bayview and community partners. This role was added to the team as a quality improvement initiative  aimed at providing urgent homecare support to vulnerable patients in the community who have recently been discharged from the Sunnybrook ED. The Occupational Therapist will complete one initial in-person or virtual home safety assessment and up to one follow-up in North Toronto Area. Patients’ functional status will be assessed, and recommendations will be made about each patient’s ability to perform functional mobility and everyday living skills. Assistive devices and adaptive equipment may be recommended and ordered to help patients manage in their homes. Personalized strategies for patients whose cognition or mood are impaired will be provided. Targeted education will be given to patients who have had a fall or at risk for falls. The OT will collaborate with community partners and the ED team to identify options for lowered risk to continue living in the community, based on each patient’s physical, cognitive and psycho-emotional functional presentation.

• Work closely with the Sunnybrook ED and ED One team
• Assessment and implementation of Occupational Therapy Interventions in a community setting.
• Assess patients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines
• Interventions include but are not limited to: Home safety equipment and mobility aids, home access modifications, falls preventions strategies, ADL Training, cognitive remediation/compensatory strategies, and linking to community resources/funding.
• Document all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the agency.
• Complete transfer of accountability to community partners to continue to follow patient in the community if needed
• Ensure that services provided are best practice, evidence based and evaluated through the routine use of valid and reliable outcome measurement.
• Adhere to the code of ethics of the profession, in maintaining standards of practice for the profession as defined by the College of Occupational Therapist of Ontario.
• Maintain and upgrade knowledge and skills through continuing education and professional development
• Seek opportunities for involvement in practice-based research and engage in knowledge translation through evidence-based practice and implementing best practices

Qualifications/Skills

• Registration with the College of Occupational Therapists of Ontario
• Membership in CAOT or OSOT
• Minimum 2 years of clinical experience required, specifically in the community
• Knowledge of the health care system, community resources and eligibility criteria for system partner facilities
• Emergency Department experience or understanding about ED processes an asset
• Strong cognitive assessment experience required
• Physical assessment experience required
• Assessment and discharge planning experience with older adults (65 years+) who have complex health issues, including failure to cope or thrive, confusion, dementia, delirium, falls, skin ulcers, weakness and decreased mobility or ambulation required
• Ability to efficiently complete comprehensive assessments of a patient’s occupational performance issues (ADLs and IADLS, home safety, cognition and perception, mobility and physical function, psychoemotional and mental health, and need for equipment/aides to optimize independence) in order to develop care plans that appropriately diverts future ED visits, prevents falls and functional decline
• Strong ability to collaborate with colleagues, as necessary
• Ability to personalize education to patients and family that prepares them to optimize safest return to functioning and perform life skills/ reengage in their roles in the community
• Good working knowledge and execution of referral systems such as RM&R and Health Partner Gateway (HPG)
• Computer literacy required
• Well-developed communication, interpersonal, time management and organizational skills
