Summary of Duties:
The Referral Coordinator is primarily responsible for providing support for the timely and efficient processing and coordination of all referrals and requests for support for PAIL Network (PAIL). The incumbent will also be responsible for providing operational support for ongoing process improvement initiatives, continuing education activities, and matching families with appropriate resources in their community.
- Liaise with referral sources and families to facilitate the efficient and timely provision of support and to maintain and improve program responsiveness to stakeholder requirements
- Foster positive relationships with volunteers of PAIL Network and community agencies
- Coordinate support needs of families with available resources both internally and externally with community partners
- Provide daily operational support for Volunteer Coordinator and Family Support Coordinator in the coordination and matching of families who are seeking support with existing groups or services (e.g. Circles of Support, Subsequent Pregnancy Group, One to One Phone Support and Web based support) to PAIL Network
- Provide operational support for the program with a focus on implementing new processes, improving established processes (e.g. referrals, intakes, scheduling, matching), and improving communication
- Facilitate the identification, implementation, and monitoring of continuous improvement initiatives related to key operational processes
- Facilitate, coordinate and monitor information sharing activities related to continuing education (teleconferencing, internal or external seminars, and discipline-specific courses)
- Participate in program development and assist in the development of new opportunities related to PAIL Network
- Maintain the PAIL database
- Data entry related to record keeping
- Additional project work related to role when such support is requested by Regional Program Manager
- Refer callers to Regional Program Manager and /or other staff when appropriate
- Other reasonable duties as assigned, including occasional weekend work and travel (within Ontario) to support events and in-person meetings
- Conduct work in a safe manner; including the prevention, identification and reporting of safety issues and problems; taking corrective action, where possible
- Adherence to privacy legislation in all record keeping , this employee will have access to confidential information related to families who are receiving support and volunteers with PAIL Network
Qualifications/Skills
- College Diploma in a health related field or equivalent required
- Provincial and/or vocational and/or professional certification or degree preferred
- Experience working with sensitive subject matter and maintaining a high level of confidentiality and previous volunteer relations practice in a community setting is preferred
- Excellent communication and organizational skills with an ability to facilitate cooperative working relationships with multiple internal and external contacts
- Interest in perinatal bereavement support
- Minimum 2 years relevant referral coordinator experience preferred
- Ability to understand and apply Hospital policy, OH&S Act, Privacy Legislation and other guidelines and legislation as required
- Excellent oral and written communication skills
- Excellent interpersonal and leadership skills
- Strong team player
- Excellent organizational, priority setting and time management skills
- Proven ability to resolve conflict and problem solve
- Ability to maintain pleasant and professional manner while dealing with multiple and competing work priorities
