Sunnybrook has an exciting opportunity for a physiotherapist to work in the Veterans Centre, to provide care to Veteran residents and transitional care patients. Physiotherapists in the Veterans Centre provide clinical care to patients/residents presenting with complex needs stemming from various conditions including those experiencing cognitive decline. The physiotherapists work collaboratively within the profession and with members of the patient care team to foster, enhance and support physiotherapy best practice and senior friendly care. In additional to clinical care physiotherapists contribute to patient/resident/family and staff education; participate in program planning, development & evaluation; evaluate physiotherapy interventions using evidence-based practice principles; act as a resource to the Department of Physical Therapy, University of Toronto; and maintain & upgrade skills and knowledge through continuing education activities.
Responsibilities include:
- As a member of the interprofessional team in Veterans Centre, the Physiotherapist will assess, plan, implement, direct, and monitor residents’/patients’ physiotherapy programs and will work closely with Physiotherapist Assistants when implementing patient and resident programs.
- Evaluation, treatment and education of conditions commonly seen in the geriatric population transitional care, long-term care and palliative care.
- Maintain and/or progressing functional mobility, for the purpose of transitioning patients to community settings
- Collaborate with interprofessional team to make and support recommendations on disposition planning and equipment and service requirements. Appropriately refer to community services as needed
- Actively participates in unit rounds and family conferences
- Maintains and upgrades skills and knowledge through continuing education activities
- Acts as a resource to hospital staff through formal and informal interprofessional education
- Act as a resource to the Department of Physical Therapy, University of Toronto for student teaching (curriculum development, clinical placements)
- Evaluates physiotherapy interventions using evidence –based practice principles
- Participates in practice –based research activities (i.e. scientific inquiry activities, and program evaluation) at the program and professional level
- Maintenance of resident records using the Point Click Care electronic documentation system, workload measurement and MDS statistics in accordance with facility and professional standards
- Participation in Veterans Centre and professional specific meetings and committees as required
Qualifications/Skills:
- MSc. [P.T.] or equivalent
- Registration with the College of Physiotherapists of Ontario
- Membership with the Canadian Physiotherapy Association of Ontario preferred
- Ability to communicate effectively, both orally and in writing, as well as demonstrate ability to work effectively as part of an interprofessional team is required
- Strong problem solving, time-management and organizational skills
- Previous experience with geriatric clients preferred
Demonstrate effective ability to:
- Assess and provide treatment for patients and residents with cognitive, behavioral and physical impairments
- Provide mobility assessments and appropriate interventions
- Foster a collaborative working relationship with OTA/PTA including providing clinical supervision through appropriate referrals, ongoing modification of goals as appropriate and providing ongoing treatment recommendations
- Provide timely comprehensive discharge planning including appropriate referrals to community services and programs
- Work collaboratively as a member of the interprofessional team
- Manage multiple priorities in the context of a large complex caseload
- Creatively problem-solve
- Provide Person-Centered Care
