Patient portal, web site and Patient Education Research Learning Centre
Summary of Duties:
The Project Coordinator will be responsible for identifying and providing a list of functionality needed from the current navigation platform E-Care (electronic care) at the Odette Cancer Centre to a new patient portal that will be implemented as part of Sunnybrook’s new Health Information System. Responsibilities will include documenting functionality on E-Care for the patient portal, providing a comprehensive list of patient education resources to be made available on the new portal, identifying any new requirements for patient education within the portal, ensuring the migration of content from current content management system to patient portal and working with the patient education team on new web site for content updates, content management and content creation. Additionally the project coordinator will have responsibilities in collaboration with the Patient Family Education Manager with oversight of the Patient Education Learning Centre operations including the training of volunteers, maintenance of education resources and triaging of patient education inquiries to the Patient Education team.
Responsibilities include:
- Audit E-care to create an inventory of functionality for patient portal lead
- Identify and document E-Care functionality required for the new patient portal
- Identify patient education resources to be available on the new patient portal
- Work with Manager of Patient Education to identify patient education functionality within the new portal
- Update and populate the portal and new web site with patient education resources
- Bring together key stakeholders and work collaboratively with clinical teams to identify patient education requirements for the new portal
- Audit E-care treatment plans to ascertain need for review and updating
- Triage system issues in E-Care with the My Chart team for resolution until the portal is launched
- Provide part time oversight in collaboration with the Patient Education Manager of the Patient Education Learning centre
- Facilitate training sessions for Patient Education Learning Centre volunteers
- Create, review and update patient education web content for the new web site.
Reports to Manager Patient Family Education – Cancer Program
Qualifications/Skills:
Preferred candidate will hold a baccalaureate degree in a field such as Health Sciences, Information Science or Business plus a minimum of 2 years related experience or the equivalent combination of education and experience
- High degree of initiative and self-directedness
- Knowledge of patient education as an asset
- Experience with content management systems like SHARE POINT
- Experience working with web based programs
- Excellent time management and organizational skills
- Excellent communication and interpersonal skills
- Work well with interprofessional team
- Ability to work accurately and efficiently while adapting to changing environment and priorities
- Computer literacy, proficiency in Microsoft Office suite of applications
