Job Summary

Primary Care Clinical Dietitian - Hybrid position in-clinic and in the community

  • Assess patient nutritional status by gaining an understanding of food habits or preferences, socio-economic and cultural backgrounds, psychological factors and medical profile
  • Conduct nutritional assessments
  • Develop, implement and evaluate individualized nutritional care plans for patients from a primary care perspective i.e., considering literacy, language, cultural, and psycho-social factors
  • Work with patients and inter-professional team members to develop and implement primary care coordinated care plans
  • Ensure practice is consistent with professional standards as outlined by the College of Dietitians of Ontario and organizational policies
  • Demonstrate a commitment to professional life long learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession
  • Perform reflective practice, analyzing and evaluating dietetic knowledge and modifying practice accordingly to improve the quality of patient care from a primary care perspective
  • Participate in Quality Improvement and program development initiatives
  • Participate in clinical case conferences and communicate appropriate patient information within the circle of care
  • Perform other duties as assigned

Requirements:

  • Bachelor’s degree of science in Nutrition or equivalent
  • Current registration in good standing with the College of Dietitians of Ontario (CDO)
  • At least 2  years of relevant experience as a Registered Dietitian in primary care and/or community based care.
  • Knowledge and ability to apply adult learning principles
  • Excellent communication, critical thinking and interpersonal skills
  • Current CPR/BLS certification
  • Ability to work with a high degree of autonomy and on inter-disciplinary teams
  • Familiarity with client-centered and self-management approaches to chronic disease prevention and management
  • Knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues on diet and nutrition
  • Excellent communication skills (verbal and written)
  • Excellent organizational and time management skills
  • Excellent problem-solving and patient service skills
  • Experience in planning and implementing health promotion programs and activities
