  • Summary of Duties

    As a member of the Assertive Community Treatment Team, the incumbent will provide client centred treatment, rehabilitation, support, and case management to clients in the community with severe & persistent mental illness.

     

    Qualifications/Skills

    • MSW, B.Sc. OT, M. Sc. OT,  or COAT approved Equivalent required
    • Member in good standing of the Ontario College of SW and SSW, or  College of Occupational Therapist of Ontario required
    • Recent community experience working with patients with severe and persistent Mental Illness
    • Knowledge and comfort with functioning within  the ACT model of Care
    • Basic knowledge of psychotropic medications
    • Ability to work independently and interdependently within a multidisciplinary team
    • Excellent oral and written communication skills
    • Demonstrated effective interpersonal skills
    • Demonstrated problem-solving skills
    • Commitment to client-centered care
    • Knowledge of the health care system and community resources
    • Valid Ontario Driver’s License required
    • Ability to travel extensively within the catchment area
