Salary Range: $44.660 - $57.900/hr

  • Summary of Duties

    The Social Worker will provide social work services to patients in the Inpatient Rehab Unit. The Social Worker will maintain the standards of practice approved by the profession. The social worker works closely with other inter-professional team members regarding the care, progress and discharge of patients.

     

    Social work intervention is a significant part of the rehabilitation program. The social worker works with patients, family members, and rehabilitation team members to identify and address person-centered goals.  The knowledge and skills of the social worker are directed toward helping the patient and family in the adjustment process and to facilitate a successful return to the community at the highest possible level of emotional, social and economic functioning. The successful candidate will complete psychosocial assessments, provide counselling, group work, locate and arrange resources, consult and collaborate with internal and external partners to achieve positive health outcomes.  This position also requires a commitment to teaching, research, administrative and professional responsibilities.

     

    Qualifications/Skills:

    • Member in good standing of the OCSWSSW
    • Previous experience working in a health care setting (preferably 2 years)
    • Excellent assessment, interviewing and problem solving skills
    • Excellent communication, negotiation, mediation and organizational skills
    • Demonstrated excellence in inter professional collaborative practice
    • Knowledge of the health care system and community resources
    • Demonstrated Practice knowledge/integration in managing/coordinating “complex care” transitions into the community
    • Superior ability to manage multiple tasks and set priorities in a fast-paced environment
    • Commitment to Inter Professional Education and Collaborative Initiatives
    • Commitment to student education
    • Demonstrated commitment to Person Centered Care approach and principles
