To provide a full range of social work services to patients and their families admitted to CVICU/B5ICU/CICU (for non-trauma/non-cardiac service patients) within the Trauma program. This includes psycho-social assessments, counselling to patients and families, end of life care, Advance Care planning and goals of care conversations, transition planning, crisis intervention, linkage to community resources, team consultation. Support in completing Trauma psychosocial assessments will also be a priority when on schedule.
Qualifications/Skills
- Masters of Social Work Degree Required
- Member of the Ontario College of Social Workers and Social Service Workers in Good Standing
- Minimum two (2) years of experience working in a Hospital setting
- Experience in critical care preferred
- Exceptional communication skills required
- Knowledge of the HCCA and SDM act
- Knowledge of complex care management and complex health system access
- Strong negotiation and conflict resolution skills
- Demonstrated skills in Inter-professional Collaborative Practice & Education
- Ability to balance collaborative practice with self-direction
- Ability to manage multiple tasks and set priorities in a fast-paced environment
- Commitment to student education
- Experience and commitment to quality improvement initiatives
- Demonstrated knowledge of organizational priorities and social work scope of practice priorities
