Salary Range: $45.910- $58.914/hr

Summary of Duties

Reporting to the Patient Care Manager, the Registered Dietician (RD) in the outpatient ALS/ Neuromuscular Clinic works within an interdisciplinary team to both coordinate and provide safe, effective and evidence-informed nutritional care to the ALS patient population. The Dietician will coordinate nutrition management for ALS patients and provide direct patient care, liaise with community supports/partners, arrange in home homecare follow up, assist with the completion of relevant forms and referrals, be familiar with various research projects and protocols, and other duties as required. The Incumbent will be passionate about this population and motivated to participate in quality initiatives, work in accordance with Clinical Nutrition Standards of Practice and support the Philosophical concepts of Person-Focused Care.  They must believe in the principles, Mission and Values of Sunnybrook; be able to provide education/counseling, and document information according to appropriate standards.

Qualifications/Skills

  • Member of College of Dietitians of Ontario (CDO);
  • Excellent collaboration, communication and time management skills;
  • Demonstrated clinical skills in caring for adults with complex needs, specifically in areas of neuromuscular disease, respiratory illness, and those with nutritional challenges;
  • Evaluate RD interventions using principles of person-centred care and evidence informed practice;
  • Minimum 1-2 year(s) experience working in a fast pace regional outpatient clinic, must adapt and be familiar with outpatient operations and flow within an interdisciplinary team setting;
  • Expert knowledge of neuromuscular disease, experience working with palliative care and multiple complex community supports, able to function effectively in an autonomous setting;
  • Minimum 1-2 year(s) experience with providing and coordinating care for patients with/requiring feeding tubes, procedures, NIV and Cough Assist;
  • Responsible for fielding patient inquiries via phone and E-mail, coordinating  patient requests for allied health referrals, providing education and teaching  re: disease progression, pre and post G-Tube procedures;
  • Ability to work effectively both autonomously and within an inter-professional team, ability to manage a diverse workload while setting priorities and meeting deadlines;
  • Experience in the clinical education of health care professionals an asset;
  • Demonstrated commitment to maintain and upgrade skills and knowledge through professional development;
  • Demonstrated computer skills e.g. Microsoft Office Suite, Windows 10;
  • Demonstrated skills with Sunnycare and Accuro;
  • Effective interpersonal and communication skills, excellent verbal and written skills;
  • Maintain health records, complete workload measurement in accordance with facility and professional standards;
  • Participate in clinical and interprofessional education activities in support of Dietetic Interns
