Salary Range: $45.9100-$58.9140/hr

This position will be for 2 days per week, from September 2025-March 2026.

 

Summary of Duties

To provide a full range of Social Work services to patients and families in the Short Term Complex Support and Transitional Care Program.  This includes psycho-social assessments, counselling to patients and families, discharge planning, crisis intervention, linkage to community resources, and team consultation.

 

Qualifications/Skills

  • Masters Degree in Social Work
  • Member in good standing of the Ontario College of Social Workers and Social Service Workers
  • Experience working in a health care setting (preferably 2 years)
  • Experience with the health care system with discharge planning and community resources
  • Familiarity with issues of capacity and consent
  • In-depth understanding of the Sunnybrook Discharge policy and its application
  • Strong negotiation and conflict resolution skills
  • Ability to be a team player as well as self-directed
  • Cognitive support experience required
  • Excellent assessment, interviewing and problem solving skills
  • Excellent written and oral communication
  • Superior ability to manage multiple tasks and set priorities in a fast-paced environment
  • Commitment to student education
