Salary Range: $45.910-$58.914/hr

Summary of Duties

To provide a full range of social work services to patients and their families admitted to the Trauma program. This includes psycho-social assessments, counselling to patients and families, discharge planning, crisis intervention, linkage to community resources, team consultation.

 

Qualifications/Skills

  • Masters of Social Work Degree Required

  • Member of the Ontario College of Social Workers and Social Service Workers in Good Standing

  • Minimum two (2) years of experience working in a Hospital setting

  • Experience in Trauma preferred

  • Exceptional communication skills required

  • Knowledge of the HCCA and SDM act

  • In-depth understanding of the Sunnybrook Policies that impact transitional planning

  • Strong negotiation and conflict resolution skills

  • Demonstrated skills in Interprofessional Collaborative Practice & Education

  • Ability to balance collaborative practice with self-direction

  • Ability to manage multiple tasks and set priorities in a fast-paced environment

  • Commitment to student education

  • Experience and commitment to quality improvement initiatives

  • Demonstrated knowledge of organizational priorities and social work scope of practice priorities
