Summary of Duties

To provide a full range of social work services to patients and their families who are admitted to Complex Malignant Heamatology. Role responsibilities include psychosocial assessments, counselling, locating and arranging resources, discharge planning/coordinating, linkage to community resources, facilitating Advance Care Planning Conversations and addressing complex care issues, and participating as a key member of the patient care team.

 

Qualifications/Skills

  • Master’s Degree in Social Work required
  • Member in good standing of the Ontario College of SW and SSW
  • Knowledge of Canadian Social Work Competencies in hospice palliative care an asset
  • Experience with grief and end of life care an asset
  • Minimum 3 years of experience working as an MSW in a Hospital; experience working with Complex Malignant Haematology patients and families required
  • Commitment to Person Centred Care Best Practices
  • Excellent communication, organization skills and time management skills
  • Strong teambuilding skills and conflict resolution skills
  • Excellent assessment, interviewing and problem solving skills
  • Knowledge of the health care system and community resources
  • Superior ability to manage multiple tasks and set priorities in a fast-paced environment
  • Knowledge of and past clinical experience in Inter Professional Collaboration and Inter Professional Education
  • Commitment to student education
  • Interest in exploring Academic Practice opportunities that address Patient and Family Care across the continuum of Health Care
