Summary of Duties
To provide a full range of Social Work services to residents, patients and their families within the Veterans Program. This includes: supporting patients, residents, and families to meet their emotional and practical needs from admission until end of life, completion of psychosocial assessments, counselling, locating and arranging community resources, consultation and collaboration with individuals and teams. Candidates must have proficient skills with admission, discharge and transition planning. Client populations served by this role include Palliative Care, Transitional Care, and Veterans.
Qualifications/Skills
- Masters Degree in Social Work
- Member of the Ontario College of Social Workers and Social Service Workers
- Experience working in a health care setting (preferably 2 years); seniors and end of life experience preferred
- Experience with the health care system, discharge planning and community resources
- Cognitive support experience required
- Excellent assessment, interviewing and problem solving skills
- Excellent written and oral communication, mediation, time management and organizational skills
- Strong team building skills
- Superior ability to manage multiple tasks and set priorities in a fast-paced environment
- Familiarity with issues of capacity and consent
- Strong negotiation and conflict resolution skills
- Ability to be a team player as well as self-directed
- Commitment to person centred care
- Commitment to student education
