Summary of Duties:

The successful candidate will have an initial assignment to the complex medical support service, a public facing transitional care area that cares for patients with complex medical needs who no longer require acute care.

 

The Patient Care Manager (PCM) is responsible for the overall leadership, management and operations of the patient care unit, clinical practice, and the quality of care.  The manager develops and sustains high performing teams that achieve operational and person centred care objectives, while creating a supportive environment for patients, families and staff. The PCM is accountable for ensuring care requirements, are met and that patient care concerns are addressed, and also works collaboratively within the Program leadership team to achieve program goals,

 

Qualifications/Skills:

  • Bachelor Degree required, Master’s degree preferred
  • Registration in good standing with an Ontario Regulated Health Professional College required, Nursing Preferred
  • Minimum 2 years acute care experience required
  • 2 Years Management / Leadership experience
  • Experience leading within a unionized environment
  • Superior leadership, communication, team building and decision making skills
  • Superior oral & written communication skills
  • Demonstrated coaching and empowering style of management
  • Demonstrated interpersonal skills
  • Proven ability to work in a team environment and collaborate with others
