Summary of Duties

  • Collaborate with patients and family to assess and improve occupational performance

  • Facilitate effective patient discharges that optimize patient safety as part of interprofessional team

  • Act as an occupational therapy expert resource for staff, students, and other healthcare providers

  • Assign components of occupational therapy to an occupational therapist assistant/ physiotherapist assistant

  • Maintain and upgrade knowledge and skills through continuing education and professional development

  • Seek opportunities for involvement in practice-based research and engage in knowledge translation through evidence-based practice and implementing best practices

  • Participate in profession-specific and interprofessional activities in support of the Sunnybrook Occupational Therapy service and the Department of Occupational Science & Occupational Therapy at the University of Toronto

  • Practice in accordance with regulatory body (COTO) standards of practice

     

Qualifications/Skills

  • Registration with the College of Occupational Therapists of Ontario required

  • Membership in CAOT or OSOT required

  • Minimum 1-year experience in acute trauma care required

  • Strong cognitive assessment experience required

  • Discharge planning experience with people who present with complex health issues, including neurological and orthopaedic trauma presentation (traumatic brain injury, responsive behaviours, post-operation delirium, cognitive deficits and mental health issues) required

  • Acute stroke experience preferred

  • Intensive Care Unit experience an preferred, as this role provides rotational consults into ICUs

  • Hand therapy and splinting experience preferred

  • Experience with person-centred assessment and recommendations related to ADLs and IADLS, home safety, driving, medication regimen, and equipment preferred

  • Demonstrated ability to manage responsive and high risk behaviors and effectively wean continuous observation

  • Superior ability to manage multiple tasks and set priorities in a fast-paced environment

  • Strong ability to optimize the patient experience in an interprofessional team environment

  • Well-developed communication and interpersonal skills

  • Commitment to clinical excellence and use of Best Practices

  • Demonstrated ability to creatively problem-solve discharge based on assessment results

  • Demonstrated commitment to Person-Centred Care approach and principles

  • Ability to demonstrate reflective practice
