Summary of Duties
The Trauma Recovery Clinic Intake Coordinator supports the needs of trauma patients after hospital discharge by providing patient navigation and completing intakes to inform required services for follow up in the Trauma Recovery Clinic. The Trauma Recovery Clinic Intake Coordinator collaborates with the Trauma Recovery Clinic Social Worker, PAA, Trauma Services Manager, and the broader trauma team, and practices in a manner that reflects the Mission, Vision and Values of Sunnybrook.
Roles and responsibilities of this role include the following:
- Telephone follow up with trauma patients shortly after hospital discharge
- Complete intake assessments using standardized form and screening tools
- Communicate and consults with trauma team members where appropriate
- Identify and initiate resources required for discharged trauma patients
- Assist with patient educational material
- Execute data collection to support program monitoring and evaluation
- Assist with other intake related tasks in the Trauma Recovery Clinic

- Demonstrated clinical expertise with trauma patients;
- Strong verbal and electronic communication skills;
- Program planning and evaluation skills;
- Demonstrated problem solving skills
- Demonstrated professional leadership and ability to be innovative and creative;
- Excellent interpersonal skills;
- Expertise in conflict management.
