$45.910/hour - $58.914/hour
Summary Of Duties:
To provide a full range of social work services to Cardiovascular Surgery, Vascular Surgery, and Cardiology patients on D3, B3SSU, C3 CICU, and CVICU. Services include psycho-social assessments, counselling, discharge planning, Advance Care Planning Facilitation, group/committee work, crisis intervention, linkage to community resources and student education.
Qualifications / Skills:
- Masters Degree in Social Work required
- Member in good standing of the Ontario College of SW and SSW
- Minimum two (2) years of experience working as a MSW in a Hospital setting,preferably in cardiovascular care
- Excellent communication skills
- Excellent organizational skills
- Superior ability to manage multiple tasks and set priorities in a fast-paced environment with a focus on transition planning into the community
- Strong Interprofessional education and collaborative skills & knowledge
- Active participation in professional education and research activities
- Commitment to Student Education and Academic Practice within a Health Care Setting
- Experience with Person Centred Care
- Ability to be flexible and respond to the program needs as appropriate
- Commitment to quality improvement
