Join our Specialized Geriatric Services team as a GEM Nurse Clinician, where you'll play a vital role in assessing and connecting frail seniors within the Emergency Department.

 

Working closely with an interprofessional team, your responsibilities will include:
•    Conducting detailed geriatric assessments, developing care plans, and implementing recommendations for seniors with complex geriatric conditions.
•    Collaborating with healthcare professionals to create and execute a comprehensive care plan.
•    Educating senior patients, their caregivers, and coaching staff on best practices in geriatric care.
•    Participating in Specialized Geriatric Services, and hospital-wide meetings, committees, and planning sessions as needed.
•    Maintaining accurate patient records and measuring workload in line with organizational and professional standards.


Qualifications/Skills:
•    Bachelor’s degree in Nursing required. Master’s degree in Nursing or Gerontology is preferred
•    Current College of Nurses of Ontario Certificate of Competence; current B.C.L.S.
•    Working knowledge of community resources, HCCSS, RGP, and specialized geriatric programs
•    Additional training and/or experience in Gerontology is required
•    Knowledge of Comprehensive Geriatric Assessment (CGA)
•    Well skilled in geriatric assessment of physical and mental illness 
•    Demonstrated ability to function on an interprofessional team
•    Excellent organizational skills, problem solving and time management skills
•    Ability to adapt to change, be flexible in an ever-changing environment
•    Excellent oral and written communication, and computer skills
